Notes and Forms

Our school provides regularly updated notes and forms for parents and students to sign.

 

Medical Forms

When a medical practitioner has prescribed medication that must be administered during the school day, parents/carers must:

  • notify the school
  • provide up to date information as required
  • supply the medication and any 'consumables' necessary for its administration in a timely way.

The administration of such medication forms part of the department's common law duty of care to take reasonable steps to keep students safe while they attend school.

The administration of prescribed medication in schools is carried out by staff who volunteer and who are trained. Except in an emergency, only individual staff members who have volunteered and been trained will administer prescribed medication to students.

Parents/carers of children who require prescribed medication to be administered at school must complete a written request (see parent request forms below). Students must not carry medications unless there is a written agreement between the school and the student's parents/carers.

A letter will be provided to parents/carers from the Principal confirming arrangements for the administration of the student’s medication at school.

Absence Forms

Following an absence from school you must ensure that within 7 days you provide your child’s school with a verbal or written explanation for the absence. However, if the school has not received an explanation from you within 2 days, the school may contact you to discuss the absence.

Absence Notes :

An absent for one day note can be used by parents/carers to advise the school when their child is absent for one day 

An Absent for part of the day note that can be used by parents/carers to advise the school when their child is absent for part of the day 

An Absent for two or more consecutive days note can be used by parents/carers to advise the school when their child is absent for more than one day.
